The Squires programs are primarily governed by the grade that a player is going into in the fall:
Middle School - Grades 7 and 8
Pee Wee - Grades 5 and 6
Mites - Grades 3 and 4
Tikes - Grade 2
Flag - Grades K, 1, and 2
To be allowed to participate, each participant must return the following completed registration documents to their head coach. A PDF version of these documents can be found at the end of this page. Use the PRINT button at the top of the PDF document to print these forms.

Page 3 - Insurance Coverage, Photograph/Name Release, and Medical Disclosure
Page 4 – Physical Form (or a copy of a doctor's physical stating the participant is cleared to play sports/football), Parent Volunteer/Buyout Form
A copy of a signed doctor's physical that clears a player for participation in their sport of choice is acceptable as an alternative to needing the squires' "Physician Clearance to Participate" section completed. physicals must never be more than 13 months since the date of exam while participating in a noble squires program. the thirteen month requirement allows parents with insurance that only covers one physical per twelve months to schedule a physical that overlaps with our requirements. Flag football and flag cheering only require a physical to be turned in the first year that the child participates. Mites, pee wee and middle school football and cheering require a physical be submitted each year of participation.
Doctor's offices are VERY busy in July/August. Be sure to schedule an appointment far in advance to ensure that your child is able to meet the requirements to participate!
Please include any special medical information that coaches should be aware of (inhalers, epi-pens, insulin, recent injuries, etc).
A copy of a notarized birth certificate (tackle football programs only!) is due before the athlete may participate in any game or scrimmage. Please black out any social security numbers shown on the copy before turning this in.
Participants must be covered under an active medical insurance policy during the entire course of the season.
